Community Specialist Palliative Care Nurse
Band 6 Community Specialist Palliative Care Nurse – South Team. Base location: Whitnash Lodge. 37.5 hours weekly. We are looking for an enthusiastic and motivated nurse with a passion for palliative care, who has recent study or training to support their knowledge and skills, and who is comfortable working autonomously and collaboratively within a multidisciplinary team. The post holder will demonstrate excellent communication skills in supporting patients and their families and will be committed to providing high quality compassionate care.
Responsibilities
* Actively manage a clinical caseload of patients requiring specialist palliative care.
* Provide specialist advice, education and training to the team and wider workforce.
* Lead and deliver changes and initiatives in management of specialist palliative care services across Warwickshire.
* Work collaboratively with the locality team to implement the specialist palliative care element of the SWFT Business plan.
* Provide specialist palliative care support across Warwickshire to meet service needs.
* Deliver teaching and education programmes.
Qualifications & Experience
* Registration as a Nurse.
* Experience in palliative or end‑of‑life care.
* Excellent communication and teaching skills.
* Ability to work independently and as part of a multidisciplinary team.
