We are seeking a motivated, enthusiastic individual for a senior support worker role.
The senior support worker will work closely with community learning disability nurses to provide all aspects of care, including assessment, planning, implementation and evaluation of care packages as designated by the multidisciplinary team.
They will support the care pathway, receive supervision and ongoing support, implement agreed care and intervention programmes, and support individuals in a variety of settings as well as in their own homes.

The Doncaster Community Adult Learning Disability Team is a specialist health team within Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) that serves adults with learning disabilities across the Doncaster Care Group.
