workers focus on what matters most to individuals, taking a holistic
approach to improve health outcomes and connecting individuals to
available services and support organisations.
The main roles of the Social Prescribing Development worker will be:
• To deliver a Social Prescribing service to adults in the areas of Alfreton, Ripley, Crich, Heanor, linking people from GP surgeries in the ARCH Primary Care Network with appropriate services and support from the voluntary, community and non-profit sector.
• To prepare a plan with adults from GPs in Ripley, Crich, Alfreton, Heanor that uses motivational interview techniques to encourage personal responsibility in the outcomes and with links to ongoing positive support from voluntary and community organisations.
• To keep records of activity which monitor your progress and that of those you support, using the Joy platform.
• To link with all health professionals and teams including social care to support adults with non-medical issues that affect their health
• Champion partnership with the local voluntary and community sector to provide comprehensive support.
• To inform and support the development of new and additional services that would be beneficial in Amber Valley
No specific qualifications are required however the role requires someone to be able to undertake a suite of national recognised training for SPLW role. Proven experience of offering practical and emotional support to adults with disadvantages. In addition, the successful applicant should have:
o A good understanding of health and social care services and of the local voluntary and community sector.
o A good understanding of the communities of Amber Valley.
o Excellent communication skills, and good literacy and numeracy.
o Good IT skills and ability to maintain records and statistics to monitor the personal progress and progress of the project using Joy platform.
o Excellent organisational, planning, time management and problem-solving skills.
o Experience of supporting adults is necessary, evidence of motivational interviewing and support within a group setting would be useful
o Ability to liaise and work with other agencies to meet needs of referrals
o Capable of undertaking training appropriate for a SPLW
