Job Title: Social Worker - Hospital (Discharge to Assess)
Location: Chesterfield Royal Hospital, S44 5BL, Derbyshire
Salary: £33-34 per hour
Contract: Full-time, Monday to Friday, 9:00 am - 5:00 pm
About the Role:
Eden Brown Synergy is seeking an experienced Social Worker to join the team at Chesterfield Royal Hospital. The ideal candidate will have hospital experience and a strong understanding of the Discharge to Assess (D2A) process. This role involves complex case work, including MCA assessments and safeguarding responsibilities.
Key Requirements:
Registered Social Worker in England
Experience in a hospital setting
Knowledge of Discharge to Assess processes
Experience with complex case work, MCA, and safeguarding
Ability to work on-site Monday to Friday, 9 am - 5 pm
