Job overview

We are looking for a forward-thinking and motivated nurse to join the Neighbourhood Nursing teams across Lambeth and Southwark areas.

We are the first Trust in the UK to trial the Buurtzorg District Nursing Model: an entirely Nurse-led approach to patient care in the community with self-managed nursing teams. You will find yourself working in a completely different way.

You must demonstrate excellent communication and team working skills. Through clinical case management you will care for patients with long-term conditions, those at risk, end of life care needs, and those with supported self-care programmes.

There will be training and opportunities in addition to clinical supervisions and appraisals. The post holder must be able to undertake a prescribing course and advanced assessment courses once in post.

Working on a 7-day shift service (hours between –, this post also includes rotational weekend work (maximum two per month).

We are currently trialling a new way on interviewing called transparent interviews, whereby we will provide the 60% of the non-clinical questions to the candidates 24 hours in advance – for further details on this please contact k – Matron for Recruitment & Retention.

Main duties of the job

The Neighbourhood Nurse participates in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions.

As a team leader, you will ensure the provision of a high standard of nursing care and health promotion.

Duties include:

1. Holistic assessment, planning, implementation and evaluation of care. For example, Chronic Disease Management, Tissue viability, Palliative Care, Nurse Prescribing, Extended and Supplementary Prescribing.
2. First contact assessment, care and referral.
3. Continuing care, rehabilitation and delivering the National Service Frameworks.
4. Providing public health, health promotional programmes to improve health and reduce inequalities.
5. Maintaining contemporaneous written and computerised records.

Essential criteria

10. Extensive repertoire of clinical skills
11. Ability to analyse and critique assessment and other information and make sound clinical decisions.
12. Ability to work autonomously and be responsible and accountable for own decisions.
13. Ability to work under pressure and to tight deadlines.
14. Ability to use network, email and use Word and simple spreadsheets and databases.
15. Able to demonstrate excellent communication skills, both verbal and written

Desirable criteria

17. Car driver with full UK licence

Experience

Essential criteria

18. Considerable post registration experience.
19. Experience of leading and managing a nursing team.
20. Experience of working as a member of a team but also able to work autonomously.
21. Experience of teaching and assessing students and other staff members.

Desirable criteria

22. Experience of working in primary care / community nursing services.
23. Experience as a clinical supervisor.

Knowledge

Essential criteria

24. Education to first degree level
25. Registered General Nurse Adult
26. Evidence of post-registration education and training in care of older people, long term conditions, palliative and end of life care.
27. Practice assessor/supervisor or old mentorship qualification or SSSAP.
28. Nurse prescriber or willingness to undertake.

Desirable criteria

29. Clinical specific post registration course wound management, diabetes, leg ulcer care, continence care and assessment
30. Specialist Practitioner District Nursing qualification.
