Are you a highly organised care professional with a passion for delivering exceptional support services? We're recruiting on behalf of a growing homecare provider for an experienced Care Coordinator to join their dedicated team. This is a pivotal role, ensuring that clients receive high-quality, consistent care tailored to their individual needsevery day.
You will be responsible for coordinating care worker schedules, managing client packages, maintaining accurate records, and supporting the Care Manager in growing and developing the service. This role offers the opportunity to step up into a Deputy Care Manager position over time.

Key Requirements:
Proven experience in care coordination and/or rostering systems (Access Care Planning software desirable)
Strong background in person-centred care, risk assessment, and support planning
Excellent organisational skills with the ability to manage shifting priorities and cover shifts where required
Compassionate, non-judgemental, and dedicated to promoting independence and dignity
Able to manage referrals, create care plans, and ensure full compliance with quality standards
Skilled in building positive relationships with clients, families, and professionals
Strong IT and administrative skills, including maintaining digital records
Full, clean UK driving licence and willingness to travel locally
Ability to supervise, train, and motivate care staff while maintaining robust communication channels
Must hold or be willing to obtain an Enhanced DBS
