Overview
The successful applicant will primarily care for patients with coronary heart disease (CHD) referred to the service after acute events (eg, myocardial infarction) or planned procedures (eg, elective PCI). They will support the Heart Failure Specialist Nurses in reviewing and caring for heart failure (HF) patients and will deliver cardiac rehabilitation to both CHD and HF patients alongside exercise facilitators.
Responsibilities
* Care for CHD patients referred following acute events or planned procedures.
* Support HF Specialist Nurses in reviewing and caring for HF patients.
* Deliver cardiac rehabilitation to CHD and HF patients in collaboration with exercise facilitators.
* Participate in professional development opportunities relevant to CHD/HF when available.
* Meet essential pre-employment requirements (produce all stated certificates) to complete checks.
Role Details
Location: County Durham and Darlington NHS Foundation Trust. Workplace at the Whitehouse in Easington. Core hours are 09:00–17:00, Monday–Friday, with no current weekend work (this may change in the future).
From 9 April 2025, Healthcare assistants require a minimum of two years NHS experience as a HCA for sponsorship consideration (sponsorship will be considered only if criteria are met and a current visa is expiring within 3 months).
