Who You Are We are looking for an experienced agency social worker passionate about supporting adults with learning disabilities. You should be eager to join a well-established and supportive team within the Rhondda Cynon Taf West Learning Disability Team for a six-month placement. What the Job Involves Your primary focus will be on undertaking reassessments and statutory reviews to safeguard and promote adults’ wellbeing. The role requires executing high-quality social work practices in assessment, care planning, and review. Daily tasks include: Undertaking assessments aligned with the Social Services and Well-being (Wales) Act 2014. Developing and reviewing care and support plans based on personal outcomes. Managing risks in a way that supports independence and individual choice. Collaborating with other professionals and agencies using a multi-disciplinary approach. Maintaining all electronic records following departmental policies and procedures. Community-based work and travel across the service area are necessary. The hybrid working model includes at least one office-based day per week. Skills Strong experience in social work practice Familiarity with the Social Services and Well-being (Wales) Act 2014 Ability to assess risks and manage care plans Proficient in collaborative and multi-disciplinary teamwork Excellent record-keeping and organizational skills Capability to travel within the service area
