Our nurse and therapist led Intermediate Care Unit offers a rewarding setting, focused on rehabilitation and reablement, with six Specialist Palliative Care inpatient beds. We use a team nursing and therapy approach, delivering person-centered care, with support workers working directly alongside qualified nurses or therapists in a collaborative environment.
This Band 3 role is ideal for experienced support staff. If you are less experienced, you can start as a Band 2 Rehabilitation Support Worker or Healthcare Assistant, with a clear development pathway to progress to Band 3 through training and supervised practice.

Main duties of the job
The post holder will be part of a dedicated team of support workers trained in a range of essential clinical and care skills. Working under the supervision of registered professionals, they will provide direct, hands-on care and contribute to delivering high-quality, person-centred care that supports both rehabilitation and end-of-life needs.
You will play an active role in helping patients regain independence, as well as providing comfort and support in the final stages of life. You will be expected to deliver delegated care and clinical interventions in line with your training and competencies.
Core clinical skills will include (but are not limited to):
* Pressure ulcer prevention and management
* Wound care
* Catheter care and bladder scanning
* Venepuncture
* Recording clinical observations, including NEWS2 and blood glucose testing
* Supporting physiotherapy and occupational therapy programmes within your scope of practice

The unit currently comprises 20 intermediate care beds for rehabilitation and 6 Specialist Palliative Care beds in a purpose-designed setting. Our aims are to reduce hospital admissions and facilitate early discharge through multidisciplinary team efforts, and to provide holistic symptom management and end-of-life care for palliative patients.

Job responsibilities

Record and report vital signs and observations, including NEWS2, skin assessments, and urinalysis.
Participate in health education and maintain competence through personal development.
Ensure accurate patient records, report changes to registered professionals, and use clinical equipment safely.

* Level 2/3 NVQ in Health or Social Care or equivalent
* Care Certificate (mandatory upon employment)
