Aftercare Worker – Trafford Council | Trafford | Full-Time Contract | Umbrella Rate: £24.31 per hour
Are you passionate about supporting young people as they transition into independent adulthood? Trafford Council are looking for a dedicated and compassionate Aftercare Worker to join their Through Care service supporting care experienced young people.
Trafford’s innovative “through care” approach ensures children and young people receive consistent, relationship‑based support from permanence through to independence. This is a fantastic opportunity to make a genuine impact on the lives of young people leaving care.
Key Responsibilities
* Carrying out the duties of a Personal Advisor under the Children Leaving Care Act 2000
* Supporting care experienced young people living independently in the community
* Building positive and trusting relationships with young people
* Working in partnership with external agencies and professionals to ensure the best outcomes possible
* Providing practical and emotional support tailored to individual needs
Qualifications
* Experience working with children, young people, or vulnerable adults
* Understanding the challenges faced by care leavers
* Excellent communication and relationship‑building skills
* Passionate about improving outcomes for young people
* Ability to work effectively both independently and as part of a wider team
