We offer our GPs:

* 12 minute appointments
* Stable clinical and administration team

Main duties of the job

The post-holder willmanage a caseload and deal with a wide range of health needs in a primary care setting,ensuring the highest standards of care for all registered and temporarypatients.


Working Relationships

All practice staff; PCN ARRS Staff and attached staff district nurses, health visitors, midwives, retinal screening.


Clinical responsibilities

In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, checking and actioning lab results and dealing with queries, paperwork and correspondence in a timely fashion.


Home Visits

Thepost-holder will be required to carry out home visits as detailed in the homevisit protocol.

Homevisits are usually carried out after morning surgery.


Duty Doctor

The post-holder will be required to participate in on-calls as part of a rota

At Plowright Medical Centre you will care for a stable population of 6,800 appreciative patients who still value general practice in the semi-rural area of Swaffham with a branch surgery in the nearby village of Necton in the heart of Norfolk in an award-winning premises.

At the same time as being traditional, relaxed,supportive and friendly, we are also highly organised and efficient, withtremendous support teams. This enables the GPs to concentrate on the work forwhich their skills are best utilised.

We are part of the Swaffham and Downham PCN, havefull support and access to a diverse healthcare team including, Health and WellbeingCoach, Mental Health Practitioners, Physiotherapists, Social prescribers,pharmacists and practice funded in-house counsellors.

We use SystmONEand have a full complement of back office staff removing some GP adminworkloads, with a very strong IT, admin and secretarialsupport

The practice receives a high QOF achievement annually andreceives great feedback from our patients.


Job responsibilities


MAIN DUTIES AND RESPONSIBILITIES

Duties may be varied from time to time under the direction ofthe Practice Manager or GP Partner, dependent on current and evolving Practiceworkload and staffing levels:


Clinical responsibilities:

In accordance with the practice timetable, as agreed, thepost-holder will make him/her-self available to undertake a variety of dutiesincluding surgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions, checking andactioning lab results and dealing with queries, paperwork and correspondence ina timely fashion

Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers within the organisation

Assessing the health care needs of patients withundifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signsof illness

In consultation with patients and in line with currentpractice disease management protocols, developing care plans for health

Providing counselling and health education

Admitting or discharging patients to and from the caseloadand referring to other care providers as appropriate

Recording clear and contemporaneous consultation notes toagreed standards

Compiling and issuing computer-generated acute and repeatprescriptions

Prescribing in accordance with the practice prescribingformulary (or generically) whenever this is clinically appropriate

In general the post-holder will be expected to undertake allthe normal duties and responsibilities associated with a GP working withinprimary care.


Home Visits

Will be required to carry out home visits as detailed in thehome visit protocol, usually carried out after morning surgery.


Duty Doctor

The post-holder will be required to participate in on-callsas part of a rota


Other responsibilities within the organisation:

Awareness of and compliance with all relevant practicepolicies/guidelines, e.g. prescribing, confidentiality, data protection, healthand safety

A commitment to life-long learning and audit to ensureevidence-based best practice

Contributing to evaluation/audit and clinical standardsetting within the organisation

Contributing to the development of computer-based patientrecords

Contributing to the summarising of patient records andread-coding patient data

Attending training and events organised by the practice orother agencies, where appropriate.


GENERAL AND CLERICAL/ADMIN DUTIES

The post-holder will be required to ensure that all relevantadministration is completed during normal working hours between 9.00 am and6.30pm.

Certain NHS and non-NHS related reports for the patients onyour list will be an expected requirement.

There is an expectation for reports to be completed in atimely and appropriate manner to meet the needs of the practices registeredpatients.


CONFIDENTIALITY AND RECORD KEEPING

Observe a strict code of confidentiality at all times.

In the course of seeking treatment, patients entrust uswith, or allow us to gather, sensitive information in relation to their healthand other matters. They do so in confidence and have the right to expect thatstaff will respect their privacy and act appropriately.

In the performance of the duties outlined in this JobDescription, the post-holder may have access to confidential informationrelating to patients and their carers, practice staff and other healthcareworkers. They may also have access to information relating to the practice as abusiness organisation. All such information from any source is to be regardedas strictly confidential.

Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the practice may only be divulged toauthorised persons in accordance with the practice policies and proceduresrelating to confidentiality and the protection of personal and sensitive data.

The post holder will participate as necessary and withappropriate cover in any training programme implemented by the practice as partof this employment, and seek to continually improve their skills and knowledge.Such training will include:

Participation in an annual individual performance review,including taking responsibility for maintaining a record of own personal/orprofessional development.

Taking responsibility for own development, learning andperformance and demonstrating skills and activities to others who areundertaking similar work.


ADDITIONAL RESPONSIBILITIES

Employment Policy & Procedures

The post holder will ensure that they read the StaffHandbook and related employment policies and procedures.

Health, Safety and Fire Regulations

The post-holder will implement and lead on a full range ofpromotion and management their own and others health and safety and infectioncontrol as defined in the practice health & safety policy, and the practiceinfection control policy and published procedures. This will include (but willnot be limited to):

Using personal security systems within the workplaceaccording to practice guidelines

Awareness of national standards of infection control andcleanliness and regulatory / contractual / professional requirements, and goodpractice guidelines

Providing advice on the correct and safe management of thespecimens process including collection, labelling, handling, use of correct andclean containers, storage and transport arrangements

Correct personal use of Personal Protective Equipment (PPE)and ensuring correct use of PPE by others, advising on appropriatecircumstances for use by clinicians, staff and patients.

Management of the full range of infection control proceduresin both routine and extraordinary circumstances (e.g. pandemic or individualinfectious circumstances)

Hand hygiene standards for self and others

Managing directly all incidents of accidental exposure

Management and advice relating to infection control andclinically based patient care protocols, and implementation of those protocolsacross the practice

Active observation of current working practices across thepractice in relation to infection control, cleanliness and related activities,ensuring that procedures are followed and weaknesses / training needs areidentified, escalating issues as appropriate to the responsible person

Identifying the risks involved in work activities andundertaking such activities in a way that manages those risks across clinicaland patient process

Making effective use of training to update knowledge andskills, and initiate and manage the training of others across the full range ofinfection control and patient processes

Monitoring practice facilities and equipment in relation toinfection control, ensuring that proper use is made of hand cleansingfacilities, wipes etc, and that these are sufficient to ensure a good clinicalworking environment. Lack of facilities to be escalated as appropriate to theresponsible manager

Safe management of sharps use, storage and disposal

Maintenance of own clean working environment

Using appropriate infection control procedures, maintainingwork areas in a tidy, clean and sterile, and safe way, free from hazards.Initiation of remedial / corrective action where needed or escalation toresponsible management

Actively identifying, reporting, and correction of healthand safety hazards and infection hazards immediately when recognised

Keeping own workareas and general / patient areas generallyclean, sterile, identifying issues and hazards / risks in relation to otherwork areas within the business, and assuming responsibility in the maintenanceof general standards of cleanliness across the business in consultation (whereappropriate) with responsible managers

Undertaking periodic infection control training (minimumtwice annually)

Correct waste and instrument management including handling,segregation, and container use

Maintenance of sterile environments
