Main duties of the job
The post-holder willmanage a caseload and deal with a wide range of health needs in a primary care setting,ensuring the highest standards of care for all registered and temporarypatients.
Working Relationships
All practice staff; PCN ARRS Staff and attached staff district nurses, health visitors, midwives, retinal screening.
Clinical responsibilities
In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, checking and actioning lab results and dealing with queries, paperwork and correspondence in a timely fashion.
Home Visits
Thepost-holder will be required to carry out home visits as detailed in the homevisit protocol.
Homevisits are usually carried out after morning surgery.
Duty Doctor
The post-holder will be required to participate in on-calls as part of a rota

We are part of the Swaffham and Downham PCN, havefull support and access to a diverse healthcare team including, Health and WellbeingCoach, Mental Health Practitioners, Physiotherapists, Social prescribers,pharmacists and practice funded in-house counsellors.
We use SystmONEand have a full complement of back office staff removing some GP adminworkloads, with a very strong IT, admin and secretarialsupport

Job responsibilities
MAIN DUTIES AND RESPONSIBILITIES
Duties may be varied from time to time under the direction ofthe Practice Manager or GP Partner, dependent on current and evolving Practiceworkload and staffing levels:
Clinical responsibilities:
In accordance with the practice timetable, as agreed, thepost-holder will make him/her-self available to undertake a variety of dutiesincluding surgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions, checking andactioning lab results and dealing with queries, paperwork and correspondence ina timely fashion
Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers within the organisation
Assessing the health care needs of patients withundifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signsof illness
In consultation with patients and in line with currentpractice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseloadand referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes toagreed standards
Compiling and issuing computer-generated acute and repeatprescriptions
Prescribing in accordance with the practice prescribingformulary (or generically) whenever this is clinically appropriate
In general the post-holder will be expected to undertake allthe normal duties and responsibilities associated with a GP working withinprimary care.
Home Visits
Will be required to carry out home visits as detailed in thehome visit protocol, usually carried out after morning surgery.

GENERAL AND CLERICAL/ADMIN DUTIES
The post-holder will be required to ensure that all relevantadministration is completed during normal working hours between 9.00 am and6.30pm.
Certain NHS and non-NHS related reports for the patients onyour list will be an expected requirement.
There is an expectation for reports to be completed in atimely and appropriate manner to meet the needs of the practices registeredpatients.
