Job overview We are seeking an experienced and compassionate Band 6 Children’s Continuing Care Nurse to join our dedicated team supporting children and young people with complex health needs across Bedfordshire and Luton. This role offers a unique opportunity to work autonomously while being part of a small, supportive nursing team delivering specialist, family centred care in homes, schools, and community settings. You will work in partnership with children, young people, their families, and a wide range of professionals to ensure safe, coordinated, and holistic care. A key part of the role involves the day to day management and coordination of our non registered staff, demonstrating leadership, effective delegation, and sound clinical judgement to maintain safe and consistent care. You will also contribute to facilitating discharges and preventing unnecessary hospital admissions through proactive community support and clear care planning. Advert The post holder will be working autonomously and in partnership with children, young people, the family, the multidisciplinary team and other health professionals within the Trust and across the health economy, as well as with statutory and third sector agencies. This post will be both across Luton and Bedfordshire. The post holder will be responsible for the day-to-day management and coordination of non-registered nurses in the Children’s Continuing Care Team. Working within a small team of qualified nurses, the post holder will demonstrate leadership and delegation skills to ensure specialist, holistic nursing care is delivered to young people safely and efficiently, whilst ensuring that continuity of care is maintained. To be part of the dedicated Children’s Continuing Care (CCCT) team who deliver care 365 days a year including unsocial hours. The main purpose of this role is to facilitate early discharge and to promote hospital aversion for the Continuing Care Team caseload of children by providing care in the home and other settings. There will be on call responsibility within a self-rostered rota to ensure support is provided to the wider Specialist Children’s Nursing Service overnight and weekends. Detailed job description and main responsibilities Main Duties and Responsibilities To assess, implement, deliver and evaluate programmes of nursing care to children and young people adopting a flexible holistic approach to meeting individual needs. Supporting children / young people and their families in meeting identified health needs, either in the family home, or other community settings as appropriate, in collaboration with other multi agency providers across BLMK. Planning – need to be able to plan and deliver evidence based skilled nursing care to children on the CCCT caseload. Be able to write comprehensive care plans. Patient contact – work in partnership with children, families and carers to provide the optimum individualised care in the most appropriate settings. Co-ordinate service provision for Children’s Continuing Care Team. You will be actively involved in practice developments and training. You will ensure that all the young people receive health care promoting a valued lifestyle, supporting independence where possible. You will help to ensure that policy and practice is developed and delivered in line with Trust and national best practice standards. The post holder will have a key role in influencing the development of specialist nursing care by sharing up-to date knowledge and skills. You will deliver planned teaching and assessment of competence for families, nurses, and non-registered professionals. You will provide the mentorship and supervision of student nurses, Preceptorship of Staff Members, and mentor Student Nurses. Supervision of unqualified staff, including safeguarding supervision. You will line manage qualified/non-registered staff, Act as a role model to junior staff members including undertaking the assessment of clinical competencies. You may be exposed to verbal and physical aggression of the young people and their families and to be able to demonstrate negotiation skills in the management of conflict across a range of situations. To be responsible for attending all mandatory and statutory training and ensuring you remain in date with all that is required. Participate in peer supervision and education sessions. Develop and contribute to a specialist area of expertise within peer review and reflective practice as part of your professional development. Ensure health records are maintained and compliant with Trust policy and keep accurate, legible, contemporaneous records and written reports as required. Report any concerns re child and adult safeguarding through the appropriate routes as per CCS guidance. Attend health reviews if requested. Be able to evaluate the effectiveness of care and in consultation with the MDT to adjust where appropriate Commitment to the Trust objectives Policy development – to participate proactively with the Team Lead to develop and improve the service. To update guidelines and Standard Operating Procedures as required. Perform aseptic non-touch technique procedures in community settings. Communication The post holder will have the ability to use excellent communication skills to establish how the health needs of children can be met by having a good understanding of child development in both health and ill health to enable the child to achieve optimum life opportunities. Communicates sensitive information concerning the child’s condition to parents/carers and young person which requires persuasive and reassuring skills. The post holder is directly responsible to the team manager and works in close liaison with the medical team, admin team and children’s specialist and community nursing teams. Provide, receive and understand complex, sensitive information relating to young people with a range of complex health needs and physical disability who frequently have life threatening and life limiting conditions. Work collaboratively with team members, team manager and other professionals at CCS to promote and ensure high standards of professional and clinical practice drawing on national best practice and Trust policy. Promote partnership working developing open and honest communication pathways with other professional teams notably hospital, social services, education and community services. This may include dieticians, GPs, Paediatricians, local and tertiary hospitals, education staff, and special needs school nurses. Able to communicate effectively using appropriate communication methods recognising barriers to communication Actively listen to parents/carers, children and young people and respond appropriately ensuring an empathetic approach is maintained. Seeking guidance and support from other professionals where needed. Know your limitations and know when to ask for support. Report and escalate as necessary any safeguarding or patient safety concerns for the young people. Attend and contribute to, team meetings, multi-disciplinary meetings, health reviews, assessments and other meetings as required by the team manager. Communicates in a way that keeps to the relevant Trust procedures and legislation such as data protection and confidentiality a sound understanding of clinical governance and clinical risk. Budgetary Responsibility Aware of service budget and supports Team Leader to stay within budget. Collects and reports relevant budgetary information as required. People Management Maintaining own personal development and requirement to take part in appraisal process. Participate in co-ordination of staff and their workload. Help others to develop in a way that keeps to legislation and Trust procedures. Maintaining own professional development and validation process. Requirement to take part in appraisal and Knowledge and Skills Framework process. Carries out 1:1’s and appraisals for other members of the team. Manages staff performance and sickness absence. Management of challenging behaviours. Research & Development Activity Participate in service audits, including service user and staff surveys and inform team of these outcomes. Dealing with complaints and compliments and escalate in accordance with Trust policy. Participate in service development and make agreed changes to role and duties as required. Clinical and Practice Governance Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes. The post holder must adhere to infection control policies and procedures of the trust. To receive and participate in child protection supervision provided by qualified staff To identify training needs and participate in personal appraisal, objective setting and review. Participate in approved research projects as appropriate. Participate in Team Development Days. The post holder must participate in clinical and other audits as required. Provide patients and their families /carers with information on standards they should expect from the team.
