*32 hour/week schedule is also available.
Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW) is required.
Position Summary
The Palliative Care Social Worker is an integral member of the Palliative Care Interdisciplinary Team, delivering patient- and family-centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize quality of life and align medical care with what matters most to the patient. Key responsibilities include evaluating psychosocial needs, providing targeted counseling including anticipatory grief support, facilitating family meetings and goal-setting discussions, making referrals to community resources, and collaborating effectively with the interdisciplinary team. This role primarily works in the outpatient clinic in Scarborough but may also meet patient and family needs at the hospital or other specialty settings through outreach (in person and/or telehealth).
This position involves evaluating psychosocial needs, providing individual, family, and group therapy, conducting complex discharge planning, coordinating aftercare services, and referring to community resources. The social worker collaborates with health and mental health professionals and provides consultation regarding social and environmental needs. Work is performed under general supervision following institutional and social work policies. Supervision of other staff or students may be required.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
1. Education: Master's Degree in Social Work/Clinical Counseling from an accredited institution.
2. License/Certifications: Current applicable state license as an LCSW or LMSW-CC.
3. Experience: Two years of relevant experience in health or mental health preferred.
