Overview

Provide physical health checks, management of health conditions advice and support, and holistic support where appropriate. Assist in the triage of referrals using local protocols to ensure timely clinical review and increased patient access to specialist advice and treatment. Support in taking detailed histories, conducting clinical assessments in partnership with patients and other professionals, recording diagnoses, tests/investigations, current medications, and contributing to the development of care plans. Support colleagues in undertaking clinical assessments as required.

To provide a phlebotomy blood sampling point of care testing (POCT) service for patients to maximise the likelihood of completing a health check within a single appointment. Be responsible for processing and management of laboratory samples requested by clinicians, ensuring correct labelling, prompt sending to the laboratory, and data entry into patient notes. Maintain and clean equipment, and restock clinical areas with a stock rotation system as agreed with the clinical team. Immediately report any safeguarding or compliance concerns. Participate in all activities required by CQC registration. Ensure skills and knowledge are kept up-to-date and evidence-based.

* Report safeguarding or compliance concerns and participate in activities required by CQC registration.
* Keep professional skills and knowledge up-to-date and evidence-based.


Qualifications and Requirements

* Experience with clinical assessment, phlebotomy, and POCT is desirable but not explicitly stated; ability to perform health checks and support care plans is required.
* Ability to work in partnership with patients and other professionals to deliver effective care.
* Understanding of safeguarding and compliance requirements.
