Burnley Community Mental Health Team - Band 6 Community Mental Health Practitioners
Burnley Community Mental Health Team are seeking to appoint two Band 6 Community Mental Health Practitioners. This post offers an opportunity for a forward-thinking and experienced individual to develop and implement safe, sound, and supportive services that embrace holistic care. Emphasizing multidisciplinary working and the principles of the Recovery Model, you will need to be an innovative thinker capable of forming effective relationships.
Requirements:
* You must be a registered mental health practitioner with excellent leadership and clinical skills.
* You should have significant post-qualification experience in clinical work with service users experiencing various mental health problems.
* Ability to work within a multidisciplinary team, assessing, planning, implementing, and reviewing care needs.
* Knowledge of the care programme approach, risk assessment and management, and understanding of how health and social needs impact mental health and wellbeing.
Key Responsibilities:
1. Undertake mental health assessments, including risk assessments, within a multidisciplinary team, working closely with the Consultant Psychiatrist.
2. Refer individuals to other clinicians or services as necessary.
3. Provide a responsive service to assess individuals' needs for access to mental health services.
4. Maintain communication between GPs, IAPT, and secondary MH services within LCFT to ensure high standards of patient care.
5. Offer mentoring and support to colleagues.
