Role Overview
With a particular focus on frailty, the Pharmacy team at Kent and Medway Community NHS Trust (KCHFT) provides specialist medicines optimisation support and guidance to patients, carers, healthcare and social care staff across community and remote settings.
Key Responsibilities
* Work on agreed Community Hospital sites and GP practices within West Kent ABC PCN, Maidstone, and remotely as required.
* Support the provision of safe and effective medicines optimisation services to cohorts of patients across these settings.
* Follow patients through the Primary Care Interface to their home when appropriate.
* Measure and monitor medicines optimisation outcomes, using specialist pharmaceutical knowledge and skills.
* Collaborate with all stakeholders to ensure medicines use is safe, evidence‑based, and aligned with patient preferences.

* Understand and improve patient, carer, and staff experience.
* Provide evidence‑based choice of medicines.
* Ensure medicines use is as safe as possible.
