* Accountable to: Head of Community and Supportive Care Services
* Working pattern: Part-time
* Salary: £27,485 to £30,162 a year
* Contract: Permanent
Responsibilities
* Work within the multi-disciplinary team and alongside experienced registered nursing staff to contribute to ongoing assessment of patients and to the provision and monitoring of care.
* Provide holistic, person-centred care and support for patients, working independently across the full range of nursing activities under the leadership of senior nursing staff.
* Promote dignity and a person-centred approach to positively enhance the lives of patients with complex life-limiting illnesses.
* Assist in monitoring patients' conditions in partnership with patients and families, escalating to a more senior nurse when required.
* Perform delegated activities in line with professional standards to provide evidence-based care.
* Assist with ongoing assessment and work with the multidisciplinary team to maintain a high standard of nursing care.
* Support patients to improve and maintain mental, physical and behavioural health and wellbeing; identify and report changes to a senior registered nurse.
* Prioritise and manage own workload, delegating safely where appropriate to other colleagues, carers, and family members.
* Communicate effectively with patients, relatives, colleagues and others, observing dignity and respect.
* Adhere to Hospiscare clinical recordkeeping policies and ensure patient confidentiality.
* Undertake safe administration and optimisation of medicines and monitor patients following administration in line with Hospiscare policy and NMC standards for Nursing Associates.
* Infection Prevention and Control: maintain a safe environment and adhere to policies, reporting risks as required.
* Disclosure and Barring Service: Enhanced DBS check with Adults and Children Barred Lists.
Qualifications and Experience
Essential
* Foundation degree
* Registered with the Nursing and Midwifery Council (NMC)
* Understanding of the Nursing Associate role within the multidisciplinary team and its contribution to service development
* Understanding and application of NMC and other professional codes of conduct, including revalidation requirements
Desirable
* Experience in specialist palliative care
* Experience working with volunteers
* Experience in providing and receiving complex sensitive information

Other Requirements
* Able to participate in a 7-day/week rota
* Able to travel throughout the Hospiscare area
* Capable of fulfilling physical and manual handling demands, including basic life support
* Able and willing to work at different Hospiscare locations occasionally
* Car owner and driver (Desirable)
