An exciting opportunity has arisen for a Clinical Pharmacist to join our team here at Wymondham Medical Partnership.
As single site GP Partnership based in Wymondham, Norfolk with 20,000 patients and 76 staff (including 4 Clinical Pharmacists), we are looking for an additional Clinical Pharmacist who is passionate about delivering high quality patient care, ensuring patient and drug safety as well as providing holistic patient centred care.
Main duties of the job
The Clinical Pharmacist will conduct structured medication reviews, long term condition reviews, deprescribing where appropriate, manage care home patients reviews, support the wider practice team with drug related enquiries, support drug monitoring processes and drug safety.
The Clinical Pharmacist will additionally support targets around the PCN DES, IIF, QOF, drug safety and monitoring searches and assist with vaccination clinics.

Job responsibilities
The following are the core responsibilities of the Clinical Pharmacist post:
Clinical Pharmacists will work as part of a multi-disciplinary team in telephone and patient facing role to clinically assess and treat patients using their expert knowledge of medicines.
They will be prescribers, be completing training to become prescribers and will work alongside the general practice team.
Clinical Pharmacists will undertake medication reviews to proactively manage people with LTC, people in care homes, frailty.
Clinical Pharmacists will provide specialist expertise in the use of medicines whilst helping to address both the public health and social needs of patients and help to tackle inequalities.
Clinical Pharmacists will provide leadership on person centred medicines and quality improvement, whilst contributing to the QOF and both practice enhanced services.
Through structured medication reviews, Clinical Pharmacists will support patients to take their medications to get the best from them, reduce waste and promote self-care.
The role has potential to significantly improve quality of care and safety for patients.
The Clinical Pharmacist will develop relationships and work closely with other pharmacy professionals across wider healthcare systems.
Clinical Pharmacists will take part in quality improvement to ensure prescribing is in line with local and national guidance, CQC requirements and up to date MHRA / CAS alerts.
Clinical Pharmacists as appropriate will support the Lead Clinical Pharmacist in mentoring and providing training / support Pharmacy Technicians.
Support the wider clinical team with education sessions and induction sessions for new clinical staff.
Support the Lead Clinical Pharmacist by ensuring appropriate representation at local ICB Medicine Management Team meetings and other PCN / Practice meetings.
