We are currently looking for a temporary part-time Care Navigator to join our friendly team. Hours of work will be as follows:
Monday - Friday 1.00pm - 5.30pm (22 5 hours per week)

As this role is a temporary role to cover maternity leave we would on this occasion prefer candidates to have General Practice/ SystmOne experience.
Main duties of the job
Suitable candidates will have excellent administration skills, be organised and have excellent communication skills and telephone manner. The role requires flexibility, willing to cover leave and to work as part of a team.
Main aspect of the role is to be the first point of contact for patients, secondary care and outside agencies. You will need a positive attitude. You will be required to provide admin support to our Clinicians by processing various tasks including prescriptions and follow up appointments whilst adhering to Practice policies and maintaining confidentiality at all times.

Bradford Student Health Service is a busy GP practice in Bradford with approximately 10,500 registered patients. We are a University practice situated in close proximity to the University of Bradford.
Care navigation is an integral part of the practice and is the patients first line of communication for all their health care needs. We pride ourselves on confidentiality, trust and respect whilst ensuring professionalism is displayed at all times. Working as a team is an essential part of working in Reception alongside admin, clinical and managerial staff.
Job responsibilities
Thefollowing are the core responsibilities of the Care Navigator. There may be onoccasion, a requirement to carry out other tasks; this will be dependent uponfactors such as workload and staffing levels:
* a.Maintainingand monitoring the practice appointment system
* b.Processpersonal, telephone and e-requests for appointments
* c.Answerincoming phone calls, transferring calls or dealing with the callers requestappropriately
* d.Signpostpatients to the correct service
* e.Initiatingcontact with and responding to, requests from patients, team members andexternal agencies
* f.Readcode data on SystmOne
* g.Photocopydocumentation as required
* h.Dataentry of new and temporary registrations and relevant patient information asrequired
* i.Inputdata into the patients healthcare records as necessary
* j.Directrequests for information i.e. SAR, insurance / solicitors letters and DVLAforms to the administrative team
* k.Manageall queries as necessary in an efficient manner
* l.Carryout system searches as requested
* m.Maintaina clean, tidy, effective working area at all times
* n.Monitorand maintain the reception area and notice boards
* o.Supportall clinical staff with general tasks as requested
