About the Role
* Work with adults with a wide range of needs, including mental health, physical disabilities, older adults, and carers.
* Complete strengths-based assessments and develop person‑centred support plans.
* Undertake safeguarding enquiries, applying professional curiosity and sound judgement.
* Work collaboratively with health partners, voluntary agencies, carers and families.
* Promote independence, wellbeing and choice, in line with the Care Act 2014.
Contribute to a positive team environment where learning and reflective practice are encouraged.

Qualifications and Experience
* Qualified Social Worker registered with Social Work England.
* Completed your ASYE and are ready for more complex, varied casework.
* Confident in applying the Care Act 2014 and related legislation.
* Strong assessment, safeguarding and risk management skills.
* Values partnership working and person‑centred approaches.
* Committed to continuous professional development and reflective practice.
