Liquid Personnel are looking for a qualified RMN/SW for our client to support with care delivery to patients referred to/receiving care from the AMHT.

Will need flexible skillset and be able to help with the following:
– Managing day to day operations for the sub team
– Supervise the clinical work of the keyworkers, monitoring caseloads and ensuring appropriate flow of patients is maintained.
– Allocate appropriate cases to keyworkers as needed
– Screen, triage and make clinical decisions about new referrals to the team
– Liaise with Hub, CRHTT, Ward etc. regarding complex cases.
– Deputise for team manager as required
– Chair regular FACT, Clinical Team, Ax, and Triage meetings during week.
