The role

We are seeking an enthusiastic and motivated Advanced Nurse Practitioner (ANP) to join our friendly primary care team. The successful candidate will primarily support our same‑day and urgent care service, seeing patients whose appointments have been triaged and allocated by a GP.

The role involves autonomous assessment, diagnosis, treatment, and management of a wide range of acute and minor illness presentations. Experience in diabetes management would be advantageous but is not essential, as support and development opportunities will be available.

The ANP will work closely with GPs, nurses, healthcare assistants, and administrative staff to provide high‑quality, patient‑centred care.

Clinical Responsibilities

  • Assess, diagnose, treat and manage patients presenting with acute and minor illness.
  • Conduct face‑to‑face, telephone and video consultations as required.
  • Manage patients allocated through GP triage systems.
  • Independently formulate management plans and provide appropriate follow‑up.
  • Prescribe safely and effectively within own scope of practice (where qualified as an independent prescriber).
  • Request, interpret and act upon investigations and diagnostic tests.
  • Refer patients appropriately to GPs, community services, secondary care, or other healthcare professionals.
  • Recognise and respond appropriately to medical emergencies.
  • Maintain accurate and contemporaneous clinical records using EMIS.

Long‑Term Condition Management

  • Support the management and review of patients with long‑term conditions.
  • Contribute to diabetes care and reviews where appropriate.
  • Work alongside the nursing team to support chronic disease management programmes.
  • Identify patients requiring proactive intervention and follow‑up.
  • Promote health education and self‑management.
  • Deliver evidence‑based care in accordance with national and local guidelines.
  • Ensure patients are involved in decisions regarding their care and treatment.
  • Safeguard vulnerable adults and children in line with practice policies.

Clinical Governance

  • Maintain professional registration and continuing professional development.
  • Participate in clinical audits, significant event reviews and quality improvement activities.
  • Adhere to practice policies, NICE guidance, CQC standards and professional codes of conduct.
  • Contribute to infection prevention and control procedures.

Team Working

  • Work collaboratively within the multidisciplinary team.
  • Provide support and advice to colleagues where appropriate.
  • Attend practice meetings and clinical discussions.
  • Contribute to service development and continuous improvement.

Person Specification

  • Registered Nurse with current NMC registration.
  • Master's level qualification in Advanced Clinical Practice or equivalent experience.
  • Evidence of autonomous clinical practice.
  • Excellent assessment, diagnostic and clinical decision‑making skills.
  • Experience working within primary care, urgent care or a similar clinical setting.
  • Ability to manage a varied workload independently.
  • Excellent communication and interpersonal skills.
  • Competent IT skills and experience using clinical systems.
  • Experience working within General Practice.
  • Experience managing patients with diabetes and other long‑term conditions.
  • Knowledge of QOF and primary care quality standards.
  • Experience using EMIS Web and Ardens.
