* Deliver person‑centred care in clients’ own homes
* Assist with personal care (washing, dressing, toileting, continence)
* Support with mobility, transfers, and safe use of equipment
* Administer or prompt medication in line with care plans and MAR charts
* Prepare meals and support nutritional needs and hydration
* Carry out light household tasks to maintain a safe, clean environment
* Provide companionship and emotional support to reduce isolation
* Observe, monitor, and report changes in health or behaviour
* Follow individual care plans and promote independence and dignity
* Maintain accurate daily records and care notes
* Adhere to safeguarding, health & safety, and infection control procedures
* Communicate effectively with families, healthcare professionals, and supervisors
* Support clients with appointments, shopping, and community access
* Respect confidentiality and comply with GDPR and CQC standards. Hours: FLEXIBLE hours of 8am to 4pm OR 5pm to 10pm (we will require you to commit to a regular shift pattern so we can fulfil the rota ahead of time)
