Crisis Resolution Home Treatment Nurse – NHS
Job Summary
Service users recover quickly at home, and as a Band 6 Nurse in the Crisis Resolution Home Treatment (CRHT) team you will play an integral part in ensuring that recovery is achieved safely and effectively as an alternative to hospital admission. You will work alongside the multidisciplinary team (MDT) to provide urgent and emergency triage, advice and assessment for people in a mental health crisis. You will also undertake home treatment visits, deliver highly skilled nursing care, conduct care planning, carry out biopsychosocial interventions, assess clinical risk and promote recovery and inclusion. You will work autonomously, attend MDT meetings, undertake gate‑keeping assessments for inpatient and crisis house admissions, collaborate with the crisis house to lead on admissions, facilitate early discharge and deliver best‑practice home‑based treatment interventions. Where appropriate you will participate in Mental Health Act assessments to explore alternatives to formal hospital admission.
Responsibilities
* Provide a clinical service that is service‑user focussed, accessible and responsive on a 24‑hour, 7‑day basis.
* Triage all mental health referrals into the team to determine priority and the appropriate clinical pathway.
* Full mental health assessment, care planning and delivery of highly skilled nursing care.
* Biopsychosocial interventions, clinical risk assessment and management to promote recovery and inclusion.
* Recommend and refer to the most appropriate team/service where treatment is indicated.
* Undertake gate‑keeping assessments for inpatient and crisis house admissions.
* Lead on care for admissions as a service in collaboration with the crisis house.
* Facilitate early discharge from inpatient wards where clinically indicated.
* Deliver home‑based treatment interventions for all clients, ensuring best practice is implemented.
* Participate in Mental Health Act assessments where alternatives to formal hospital admission are considered.
* Provide clinical leadership and supervise junior staff.
Person Specification – Essential
* Registered Mental Health Nurse with current professional registration with the NMC.
* Demonstrable post‑registration experience as a Registered Mental Health Nurse.
* Professional experience of working in mental health and MDT working.
* Experience coordinating and planning inter‑agency approaches in mental health care.
* Experience assessing and managing risk.
* Evidence of Continuing Professional Development.
* ENB 998 or other NMC approved mentor qualification.
* Key skills in literacy, numeracy and IT Level 2 or equivalent.
