Mental Health Practitioner – Trafford Neighbourhood Team (Living Well)
Join us as a Mental Health Practitioner to support the Trafford Neighbourhood Team (Living Well) based at the Wellbeing Centre, Old Trafford. The role covers North Trafford with an expectation of working across other hubs – Central, West and South Trafford – as service needs arise.
The postholder will undertake a key worker role while also conducting assessments and formulating care plans within a multidisciplinary team. The Living Well service provides timely access to mental health support for individuals who may not meet the traditional thresholds of other services, working closely in partnership with Bluesci, our third sector partner.
Key Responsibilities
* Act as a key worker, providing direct support across multiple hubs.
* Conduct assessments and formulate care plans for clients.
* Triage and signpost referrals to appropriate agencies and professionals.
* Liaise with a broad range of community services and third‑sector organisations in Trafford.
* Participate in CPD opportunities and support colleagues’ personal interests and skills development.
Essential Qualifications & Experience
* Experience in triaging and signposting mental health referrals.
* Strong knowledge of community services and third‑sector organisations within Trafford.
* Ability to work collaboratively with multidisciplinary teams.
* Trauma‑informed, recovery‑focused approach to care.
