Who You Are We are seeking a skilled and proactive Occupational Therapist who is committed to promoting independence and safety. You will be an integral part of our Adult Social Care team, collaborating with service users, carers, and partner agencies to deliver impactful solutions. Your goal is to transform lives through practical approaches and support systems. What the Job Involves Complete comprehensive functional assessments as per the Care Act Participate in triaging, screening, and signposting incoming referrals Design and recommend major adaptations like level-access showers and ramps Prescribe and arrange necessary equipment and adaptations Undertake manual handling risk assessments and prepare handling plans Coordinate with contractors, housing teams, and grants officers Maintain accurate records and generate professional reports Deliver interventions in compliance with legislation and standards Contribute to service development and represent the service in a team setting Skills HCPC registered Occupational Therapist Experience in community-based social care settings Knowledge of housing adaptations and manual handling Familiarity with Disabled Facilities Grants and legislation (preferred) Experience working with contractors and housing teams (preferred) Proficiency in electronic case management systems (preferred)
