BST/Community Nurse Team Leader - Wolverhampton

An exciting opportunity for a passionate, committed and experienced leader to join the Community Learning Disability team in Wolverhampton as a Behaviour Support team & Community Nurse Manager. The role requires strong leadership skills and an understanding of support needs of people with learning disabilities and other co‑morbidities which may include challenging behaviour and mental health needs. The team manager will work in collaboration with multidisciplinary team members and be committed to high quality health outcomes for people with learning disabilities in Wolverhampton.
Main duties of the job
* Accountable to the Service Manager for first‑line management and coordination of BST/Community Learning Disability Nurses.
* Proactively lead and manage staff to provide personalised care and support to people with learning disabilities who have complex needs.
* Ensure effective liaison with other services to enable a multidisciplinary team approach.
* Responsible for implementing service development, resource allocation and policy development.
* Ensure team activity aligns with performance parameters and quality standards.
* Supervise and appraise team members.
* Evaluate clinical care for effectiveness and promote optimum health and wellbeing, meeting agreed outcome measures.
* Provide specialist learning disability nursing services for adults over 18 with complex needs and/or mental health problems and their carers.
* Assess individual care needs and design appropriate interventions in the treatment of adults with learning disabilities.
* Teach and mentor nursing and non‑nursing staff.

Management
* Provide leadership, direction, motivation and guidance to the team.
* Delegate work appropriately to staff of differing skill and grade.

* Seek appropriate clinical supervision.
* Provide managerial supervision to team members.
Leadership
* Practice within the NMC Code of Professional Conduct.
* Develop the workforce and contribute to service redesign, development and delivery.
* Represent Community Learning Disability Nurses at organisational forums.
* Design and implement clinical care pathways and protocols for continuous service improvement.
* Involved in safeguarding adults and promote learning disability nursing within the wider health and social care economy.
* Lead the team in clinical audit and risk management.
* Acknowledge accountability for efficient caseload management.
* Set and review the highest standards of care within available resources, using audit, supervision and appraisals.
* Promote health education and health promotion.
* Ensure efficient requisition and safe use of accommodation, equipment and consumables.
* Participate in complaints, concerns and incident investigations.
* Encourage improvement suggestions in liaison with the Service Manager.
* Assist in recruitment and selection of staff with the Service Manager.
* Undertake risk assessments and report findings; contribute to adverse incident reporting.
* Advise on local and regional policy, strategy and operational practice regarding learning disability health needs.
Person Specification
* NMC Registered Nurse – Learning Disabilities.
* Experience managing and supervising staff.
* Experience working within a community setting/team.
* Minimum 2 years post‑registration experience.
* Experience with multidisciplinary team (MDT) work.
