Job Description:
We are seeking a Service Coordinator to work cooperatively with individuals and their families to identify goals and locate support services that enhance well-being. The role involves collaborating with healthcare providers, insurance, natural supports, and community services to ensure coordinated care.
Responsibilities include:
1. Supporting and coordinating care needs with participants, supports, and providers.
2. Facilitating team meetings and communication among all parties.
3. Documenting encounters and follow-up activities.
4. Maintaining participant eligibility for the waiver program.
5. Facilitating referrals to social and medical services.
6. Completing required documentation within regulatory timelines.
7. Ensuring participants maintain active Medicaid and benefits management.
Minimum Qualifications:
* Bachelor’s degree in a Health and Human Service field.
* Three years’ experience in Care Management with a Bachelor’s, or one year with a Master’s degree.
* Experience with mental health, substance abuse, and physical health co-occurring issues.
* Knowledge of community resources and Medicaid services.
* Valid New York State Driver's License.
* Strong communication, organizational, and collaborative skills.
* Computer proficiency, including Windows OS and document management.
Physical and Working Conditions:
Includes occasional walking, standing, lifting up to 30 pounds, and indoor office environment with temperature fluctuations.
Supervisory and Safety Responsibilities:
Performs within established guidelines, assists in training, and adheres to safety policies, including emergency procedures and confidentiality.
