Assistant Patient Pathway Coordinator (CAT 8 - Renal) - Band 3

We are seeking a full-time Assistant Patient Pathway Coordinator to support the Patient Pathway Manager and Coordinators by executing delegated operational activities. The role requires working autonomously, using initiative, and applying specialist knowledge to make informed decisions.
The post holder will serve as the primary contact for administrative issues related to patients' pathways of care. Working within a multidisciplinary team, they will support the proactive management of patient pathways from referral to discharge, ensuring smooth processes.
The role emphasizes high patient and clinician satisfaction by maintaining accessibility, customer focus, and expertise. Standard Operating Procedures (SOPs) will guide how responsibilities are carried out, including adherence to specified timescales.
Main duties of the job
1. Specialize in appointment bookings for specific modalities, with an overarching knowledge of all booking processes.
2. Ensure compliance with scheduling guidelines, protocols, and clinical targets, addressing breaches and managing timetable changes impacting capacity.
3. Proactively schedule new and follow-up appointments, optimizing capacity and ensuring patients are seen by the appropriate clinician first time.
4. Verify that diagnostics and investigations are requested and results are available for consultations, including those conducted at other Trusts.
5. Coordinate appointments and procedures across other hospitals and organizations as required.
6. Complete and process clinic cancellation forms per SOP.
7. Reschedule outpatient clinics in line with waiting time targets.
8. Utilize the Trust's medical transcription system to process correspondence according to SOPs.
9. Collaborate with patient records, clinical teams, and external organizations to ensure all medical notes, referrals, results, and discharge summaries are available for appointments and consultations.
