Our Client, Manchester City Council, is seeking an Adult’s Social Worker to join their Transitions Team.
This is a Hybrid Working Role!

Responsibilities of this role:
* Assess young people’s needs as they move from children’s to adult services
* Plan and coordinate smooth transitions (Care Act–led)
* Work with families and multi-agency partners
* Support independence, choice, and outcomes
* Safeguard vulnerable young adults
* Complete care/support plans and reviews
* Ensure legal compliance (Care Act, MCA)
Requirements of this role:
* Must be a Qualified Social Worker, registered with Social Work England.
* Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
* Must have minimum of 3 years experience working as a Qualified social worker
