We are looking to appoint a STAR Trusted Assessor.
18.5 hours per week
Your duties will include:
* Ability to carry out comprehensive assessments in hospital and community settings across the Borough of Sandwell
* Identify individual needs and personal outcomes through thorough assessment
* Develop personalised support plans aimed at meeting identified needs
* Complete appropriate risk assessments to identify potential risks
* Implement actions and strategies to minimise any risks identified
Essential requirements for the role are:
* Minimum of 1 years’ experience working with vulnerable adults and carers
* Knowledge of assessment frameworks, including risk assessment
* Ability to travel throughout the borough as required
* Availability for 7‑day working, including some unsocial hours
* GCSE English and Maths (or equivalent qualification)
