Responsibilities
* Provide specialist paediatric dietetic assessments and treatment interventions to children and families within health centres, children’s centres and other locations as appropriate, within Merton.
* Liaise with other agencies and healthcare professionals and maintain accurate clinical records as per the clinical record keeping policy.
* Build strong networks with GP’s, NHS Health Care Teams, Health visitors, School nurses, and other referral agencies and independent contractors to facilitate appropriate referrals.
* Work with the clinical lead and medicines management team to implement paediatric appropriate prescribing initiatives across CLCH.
* Initiate and participate in clinical governance activities/audit within the Integrated Complex Needs Team.
* Ensure community health care teams and non-NHS organisations have access to up to date nutritional information through development of education and training sessions.
* Act as a source of expertise to members of Integrated Complex Needs Team and other Health Professionals within CLCH.
* To be able to read, action and implement dashboard/service data to affect timely patient intervention under the direction of the B7 paediatric dietician.
