Job summary

An exciting opportunity has become available for a Band 7 Physical Health Practitioner to join Auckland Park Hospital, an MHSOP organic service.

We are looking for an enthusiastic and dynamic individual who is able to work with the wider MDT and external parties.

Wellbeing of patients and colleagues and working collaboratively is essential as we see this as an important aspect to keeping our staff happy at work.

The team aim is to work in partnership with patients, carers and others utilising the various services and teams we network with to work towards achieving person centred goals.

Main duties of the job

Promotes and maintains safety, privacy, and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.

Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act.

Responsible for recognising the deteriorating patient, complex morbidities and the potential for or signs of patient harm, abuse or neglect, reporting all such concerns and taking all reasonable steps to protect the patient.

Responsible for ensuring the highest professional standards and attitudes towards the care of patients are always maintained and that care is delivered in accordance with evidence-based practice, the 6 Cs and the Trust's Values and Behaviours.

Assesses patients including those with complex needs and develops, implements, and evaluates intervention plans as part of the multi-disciplinary team.

Plans and co-ordinates clinical interventions to challenging behaviour such as self- harm and aggression.

Demonstrates safe moving and handling of patients using equipment as required.

Provides training and development to all members of the clinical team, monitoring and evaluating learning outcomes, providing feedback, and assessing competencies to facilitate learning in practice.

Maintain up to date knowledge and competency in the skills required to perform safely and effectively in the role. Undertake relevant training (including statutory and mandatory training).

Job description

Job responsibilities

The successful applicant will work with the MDT to support and care for a caseload of patients offering 1:1 interventions, diagnostic assessments, admission processes, MDT care review meetings, frailty and falls reviews and leading and supporting venepuncture, physical assessments and ECG training and practice. Also working closely with occupational therapists, physiotherapists, dieticians, speech and language therapists and psychologists to ensure patient recovery and directions are supported.

Qualifications

Essential

1. First level Registered Nurse Adult (RNA) or Registered General Nurse (RGN) with current NMC registration and eligible to practice, or Registered Allied Health professional (AHP) with current HCPC registration and eligible to practice
2. BSc Nursing (or AHP equivalent), or post-registration specialist course at BSc level
3. Post-registration teaching and assessing qualification (. Mentorship, ENB 998 or City and Guilds 730)
4. Post-registration Physical Assessment Skills (Adult) Level 6 qualification
5. Independent prescribing Level 6 qualification (or willing to work towards within 12 months)
6. Evidence of continual professional learning and development

Desirable

7. Registered Mental Health Nurse (RMN) or Registered Nurse Learning Disabilities (RNLD) with current NMC registration and eligible to practice
8. MSc Nursing (or AHP equivalent) or post-registration specialist course at MSc level
9. Post-registration Advanced Physical Assessment Skills and Disease Management Level 7 qualification (or equivalent
10. Advanced Pathophysiology Level 7 qualification (or equivalent)

12. Extensive clinical knowledge and experience
13. Substantial experience of working clinically at Band 6 or above
14. Ability to assess and implement a range of evidence based therapeutic care interventions

17. Demonstrates autonomous working and management of own caseload
18. High level of personal and professional motivation

31. An understanding of clinical governance and the application of the clinical governance agenda in the NHS
