Service Care Solutions is supporting a reputable non-profit provider, currently recruiting for a Case Manager (also known as a Recovery Worker) to join their team on a permanent basis.
This service has supported individuals with addictions for many years and prides itself on being innovative and forward-thinking. It works closely with recovery communities to provide flexible, responsive services aimed at achieving the best possible recovery outcomes for all service users.
We are looking for a dedicated individual with experience working with service users with addictions.
The Role of a Case Manager
To demonstrate expert and effective practice around substance misuse treatment. Provide advice, brief interventions, some structured key working, assessments, and a needle exchange service.
Key Responsibilities for a Case Manager
* Undertake comprehensive assessments to evaluate the eligibility, suitability, strengths, and needs of service users.
* Devise, monitor, review, and update recovery care plans to ensure needs are met promptly and appropriately.
* Deliver care packages, including psychosocial and psychological interventions where appropriate.
* Conduct risk and need assessments, and implement risk management and care plans in line with policies and procedures.
Essential Requirements for a Case Manager
* Relevant Degree or Level 5 qualification (e.g., Social Work, Psychology), or
* Diploma in Substance Misuse or equivalent, and
* Level 3 Drug and Alcohol qualification or equivalent.
