Responsibilities
* Provide urgent community response services as part of our integrated team.
* Work collaboratively within an MDT including nurses, occupational therapists, physiotherapists, advanced clinical practitioners, assistant practitioners, and healthcare assistants.
* Assess, treat, and evaluate client needs in community settings, offering short-term interventions to prevent hospital admissions.
* Build complex relationships with stakeholders and network effectively to enable service integration.
* Share skills and work closely with team members to deliver comprehensive, MDT-driven care using the Calderdale framework.

* Enjoy working as part of a multidisciplinary team.
