ARBD (Alcohol Related Brain Damage) describes symptoms such as difficulty remembering, concentrating, feeling confused, irritability, or difficulty making decisions.
Milestone is a step-down unit for people with ARBD who no longer need medical intervention but are currently in an acute hospital setting and cannot go directly home with support. Penumbra Milestone is a 10-bed short-term care and recovery service (approximately 12 weeks) for vulnerable men and women aged 18+ diagnosed with ARBD.
The service is alcohol-free and promotes recovery through group work and 1-1 support, using our specialist ARBD Toolkit, Penumbra’s HOPE toolkit, and IROC (Individual Recovery Outcomes Counter). Our goal is to help residents build resilience, develop coping strategies, and minimize relapse, increasing their chances of prolonged recovery. During their stay, residents benefit from the expertise of our multidisciplinary team, including Occupational Therapy, Physiotherapy, Psychology, Social Work, Psychiatry, and visiting professionals.
The service aims to reduce unnecessary hospital stays for ARBD patients by addressing their holistic needs, focusing on recovery, rehabilitation, and re-enablement, leading to positive outcomes for residents, families, communities, and health services.
The service is a partnership between Penumbra Mental Health, NHS Lothian/Integrated Joint Boards, and City of Edinburgh Council. Referrals come from hospitals and the regional alcohol detox unit. The staffing includes:
* Penumbra: Management, Registered Nurses, Mental Health staff
* NHS Lothian: Clinical Lead, Psychiatrist, Psychologist, Occupational Therapist, Physiotherapist, Project Management
* City of Edinburgh: Social Worker, Social Care Assistant, Mental Health Officer
