Local Area Coordinator
Cheshire East Council is seeking a Local Area Coordinator to support Adult Social Care teams by identifying resources, groups, services, and organizations within the Cheshire East area that will benefit adults with Care Act eligible needs following assessment. The role involves coordinating information on available community support within the Cheshire East Council (CEC) area, informing community teams and the LiveWell site.
The key responsibilities include enabling and supporting adults with Care Act eligible needs, as well as those on the margins of eligibility, to access existing community support instead of relying solely on funded CEC services. Additionally, the coordinator will undertake assessments when required and identify low-level equipment, assisted technology, and access to reablement services.

Hours are 30 per week, preferably over four days, with a hybrid working model combining office and home-based work. The role requires an enhanced DBS check.
