Salary: £29,540 – £31,022 plus 1.81 % weekend enhancement
Location: Huddersfield Royal Infirmary, Acre Street, Lindley, Huddersfield, HD3 3EA.

Core responsibilities include:
 * Providing initial assessments, reviews, and safeguarding enquiries for vulnerable people and their carers in the hospital.
 * Arranging meetings with service users, family members, and carers to identify care needs, completing person‑centred assessments and sourcing relevant services.
 * Attending multi‑disciplinary meetings and facilitating safe discharges with support from Deputy Team Managers.
 * Supporting the team during periods of high pressure to ensure medically fit service users are discharged safely and timely.
 * Providing additional therapy support to enable users to regain independence or a short stay in a Recovery Bed.
Key qualifications and skills:
 * Experience in a social care environment providing information, advice, and guidance.
 * Knowledge of relevant Adult Social Care legislation and equality issues.
 * Ability to communicate with people who have a wide range of complex needs.
 * Strong literacy and numeracy, with record‑keeping and financial assessment skills.
 * Teamwork in a multi‑disciplinary setting and capacity to build professional relationships.
 * Full and valid driving licence and ability to travel using a council vehicle or own car.
 * Willful DBS check; a criminal conviction does not preclude appointment if considered suitable.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.
