The Advanced Clinical Practitioner (ACP) will work as an integral member of the general practice multidisciplinary team, with a primary focus on acute, on‑day demand, alongside some long‑term condition (LTC) management and home visits.
Responsibilities
* Assess, diagnose and manage patients presenting with acute problems on the day within an agreed scope of practice and competence.
* Provide some planned reviews for stable long‑term conditions and selected home visits, as agreed in the job plan.
* Work autonomously but with readily available GP supervision, escalating complex or high‑risk cases appropriately.
* Help manage GP workload safely and effectively, while ensuring that GPs retain responsibility for undifferentiated, complex and high‑risk presentations.
* Contribute to a supportive teaching and learning environment within the practice, supporting the development of future clinicians.
Role Context
This role is complementary to, and not a substitute for, GPs. GPs remain responsible for overall clinical leadership and holistic management of complex patients.
The practice hosts two GP registrars and undergraduate medical students.
Team and Support
Talybont Surgery is a friendly, progressive, partner‑led practice committed to delivering high‑quality patient care within a supportive multidisciplinary environment. The clinical team currently consists of six GPs (four salaried and two partners), two GP registrars, two prescribing pharmacists, a mental health practitioner, a women’s health specialist nurse, two practice nurses, and a healthcare assistant (HCA). The team is supported by an experienced and friendly administrative, reception and management team.
