Wehave an excellent opportunity available for enthusiastic and motivated ClinicalPharmacist to join our team. Working within this team, you would have theopportunity to work alongside GP Practices within the Hurley Group.


Main duties of the job

This role will support the medicines management aspects of thisinitiative through working within the general practice setting with the supportof a Band 8a Clinical Pharmacist. Thisrole will work as part of the general practice team to provide specialistadvice to patients, and undertake full patient centered medication reviews toproactively manage people with complex polypharmacy, particularly the elderlyand those with multiple conditions.

Work in general practice will also involve medicines reconciliation,management of prescribing queries and acute medication issues followinghospital discharges, and the development and implementation of safe systems formedication ordering and prescribing in the context of improving safety andreducing waste.

The role within the Hurley Group will also involve working with theeConsult (an online remote consultation tool https://econsult.net ) to process patientqueries. Some of this work will be done in the eHub, a central hub thatprocesses e-consultations across 11 primary care sites in London. The e-Hubutilises remote prescribing via EPS v2 enabling fewer visits for patients totheir GP practice.

Job responsibilities

MAIN DUTIES & RESPONSIBILITIES

POLICY/SERVICEDEVELOPMENT

* Work within the eHub to process medication eConsults,reconcile medications and process medication workflow across the HurleyGroup primary care sites in a virtual online setting.
* To work with general practice staff, on the ground, topromote and embed the role of a GP practice pharmacist, and raiseawareness of services available to patients.
* To provide general practice staff with recommendationsand plan action to proactively improve the quality of systems andprocesses to improve medicines optimisationwithin the general practice setting.
* To provide trainingregarding medication related issues and policies/guidelines to generalpractice staff in order to promote medicines optimisation, repeatdispensing and high quality cost-effective prescribing as appropriate.
* To provide operational and clinicalsupport to medicines optimisation developments and prescribing initiativesas dictated by GP practice, CCG, and hospital Trust and serviceobjectives.
* To collate and feedback on electronic dischargerelated information to ensure continuing improvement of the hospitalcomputerised discharge letter and process of communication.
* To assist with the implementationof policies and guidelines to improve medicines optimisation acrossprimary/ secondary care interface and into the general practice setting.
* To provide trainingto general practice staff to promote medicines optimisation in practice.
* Develop and deliver patient facing services andclinics which improve patient outcomes, reflect the needs of the practicepopulation and shift workload from other disciplines
* Deliver high quality medication reviews and developbespoke medicines treatment plans for patients (particularly the elderly,complex patients and those at high risk of admission) to reduce risk andunplanned hospital admissions
* Contribute to the achievement of KPIs and objectivesof the programme
* Work with the general practice team to ensure thepractice is compliant with CQC standards, leading improvement initiativeswhere medicines are involved

PATIENT/CLIENTCARE

* To actas a specialist practitioner in GeneralPractice
* To promote and deliver pharmaceutical carein general practice and promote safe pharmaceutical management across theinterface
* Provide pro-active leadership on medicines and prescribing systemsto the practice multidisciplinary team, patients and their carers
* To provide highly specialised evidence-based clinical advice andinformation to nursing and medical staff on the care and treatment ofpatients
* To actively participate in key multi-disciplinary ward rounds.
* Toliaise with other pharmacists to ensure efficient transfer of clinical andmedicines management issues
* Contribute to patient safety culture through Significant Event Reportingand investigation of medicines related incidents and through pro-activemeasures
* Ensure that patients are referred to the most appropriatehealthcare professional for the appropriate level of care within in anappropriate period of time

FINANCIAL/RESOURCE MANAGEMENT

* To identify areas of potentialcost-savings and to implement systems to realise savings and containprescribing cost
* To follow policies and guidelines tocontrol drug expenditure and ensure cost-effective prescribing.
* To educate other healthcare professionalson the costs associated with drug therapy and promote the use ofevidence-based, cost-effective prescribing within the division

STAFFMANAGEMENT & TRAINING

* Actively participate in national training scheme for clinicalpharmacists
* To participate in the in-serviceclinical pharmacy training of all pharmacy staff.
* To undertake CPD to ensure thatknowledge is continually updated
* To participate in the training anddevelopment of medical, nursing and other healthcare staff, as appropriate
* To actively develop an Advanced PracticePortfolio mapped to the RPS Advanced Practice Framework with a view to RPSFaculty recognition
* To act as a role model for pharmacistsdeveloping advanced practice skills

RESEARCH & DEVELOPMENT

* To regularly initiate or contribute torelevant practice research or audit projects and to publish the results ofsuch work, as appropriate.
* To participate in other research andaudit programmes in the department as required
* To maintain and further developspecialist information on the use of drugs in clinical conditions relevantto the specialty.
* To publish and present the results andoutcomes of own research and audits locally, nationally or internationallyas appropriate


Person Specification


Experience

* 2 years qualified with appropriate post registration experience in hospital or GP practice pharmacy or equivalent area of practice with assessed competencies
* Previous clinical pharmacy experience in relevant area
* Previous clinical pharmacy experience in Acute and/or GP Practice
* Experience of supervising others
* Experience of practice research or audit
* Experience of protocol/guideline
* Experience of delivering training
* Experience of directorate work
* Willingness to develop advanced practice using evidence mapped to the RPS Advanced Practice Framework


Communication Skills

* Be able to provide complex information on medicines use, clinical pharmacy and financial issues to senior consultants and managers
* Be able to impart knowledge on medicines use to other healthcare professionals
* Be able to communicate effectively in both written and verbal formats with staff and patients
* Be able to influence junior medical staff
* Be able to use presentation aids
* Track record of successful team working
* Able to motivate self and others


Qualifications

* Registered Pharmacist MRPharmS
* Mandatory CPD to maintain fitness to practice
* Postgraduate Certificate in Clinical Pharmacy or Pharmacy Practice (or equivalent)
* Working towards a Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice (or equivalent)
* Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice
* Member of the RPS faculty
* Independent Prescribing Qualification in which accreditation will be used to support management and outcomes for patients
