An exciting opportunity has arisen for a highly motivated and proactive individual to contribute to the coordination of care provided to Cancer patients.
Chesterfield Cancer Pathway Team is looking to appoint an additional part-time Cancer MDT Coordinator and Pathway Tracker to join our expanding team. You will be joining at an exciting time as we continue to grow our Pathway Team.
Our cancer pathway team tracks all suspected and diagnosed cancer patients, ensuring they are seen and treated promptly within national cancer waiting time targets. We support weekly multidisciplinary team meetings, and as an integral member of these teams, you will manage the pathway accurately, including preparing and collating clinical information and meticulously capturing cancer data for national clinical audits. Previous experience is not essential, as training will be provided.
As a member of the Cancer Pathway Team, you will collaborate with members of the Cancer Multidisciplinary Teams and administrative staff:
1. Track and proactively manage suspected and diagnosed cancer patients from referral to either non-malignant diagnosis or first and subsequent treatments, ensuring national waiting time standards are met.
2. Support the Cancer Management Team and facilitate MDT meetings for specific cancer sites.
3. Assist in data collection for suspected and diagnosed cancer patients and enter this information into the appropriate database.
4. Provide breach reports for local and tertiary cancer teams as required.
5. Facilitate video conferencing to ensure timely and accurate transmission of discussions and data between participating sites.
You will need excellent communication skills, as you will interact with clinical and administrative teams within and across Trusts.
