Social support required at home and to go out in the community. Would like to attend the temple, go shopping and for walks to the park.
Main Diagnosis
Stroke, weakness in hands, reduced mobility, type II diabetes
Communication
Gujarati is first language, can speak limited English
Moving and Handling
For short distances will use a Zimmer frame and rollator, has a wheelchair for longer distances.
What You Enjoy Doing
Used to attend Yoga until Stroke, likes being active. Enjoys going to temples, shopping, parks for walks.

Driving License Required
YES, mileage will be paid by employer
Certification
An enhanced DBS certificate is applicable to this role. DBS check applicable.
