To provide high quality nursing care to patients on the Sutton Health & Care (SHC) Reablement Unit as part of a cohesive Multi-DisciplinaryTeam. The post holder is responsible for holistic assessment and care needs of patients, including planning, implementation and evaluation. Inaddition, the post holder will be involved in health promotion and innovative activities thatenhancepatient servicedelivery andthe patient experience. The post holder will at times have management responsibility such as Nurse In Charge and supervising junior members of staff.
This post is not suitable for a newly qualified nurse.

Detailed job description and main responsibilities
Clinical
·The post-holder will ensure that in all areas of practice they adhere to all current NMCguidance,andmustmaintainaccurate,comprehensiveandcontemporaneousrecordsinaccordancewithTrustandNMCguidelines,ensuringthatconfidentialityis maintained at all times
·Takeresponsibilityforagroupofpatientsanddeliverhandsoncarewithinownlevelofcompetence.
·To give skilled nursing care, based on principles of best practice and clinical evidence.and ensure that nursing care is patient focused and patient dignity and privacy aremaintainedat alltimes.
·Undertake comprehensive assessment of the physical, psychological and social careneedsofpatientsandprovidepersoncentredcareplansthatfacilitatethepatient’sownchoices,promotingindependenceand selfcare,as appropriate.
·Toensurethesafehandlingofbodyfluidsandcontaminatedsharps.
·Be alert to the needs of vulnerable adults including risk assessment and taking appropriateactionwhen required.
·Safelyadministerprescribedmedicationsandtomonitortheireffects.
·Usereflectivepracticeasamethodofensuringthatappropriateeffectivenursingcareisdeliveredto eachpatient.
·Ensurecompetencyandsafemaintenanceofallmedicaldeviceswithinthesphereofresponsibility.
·Workwithnursingandother healthcareprofessions,patientsandtheirrelatives/carerstoassist in the timely discharge from the clinical area through appropriate dischargeplanningin conjunction with theDischarge Co-ordinators.
