Are you a registered Physiotherapist ?

Our Rapid Response & Intermediate Care service is expanding across Buckinghamshire – and we need you to join our Urgent Community Response squads.

1. Your Role : To provide expert clinical interventions and rehabilitation as part of a dynamic, multidisciplinary squad that reacts quickly when people are in crisis. Our target response time is under 2 hours
2. Your aim: To help patients stay in their own home, empowering recovery and preventing unnecessary conveyance or hospitalisation.
3. Your team : Add your expertise to our multi-disciplinary community team working with Physiotherapists, Occupational Therapists, Nurses, Specialist Paramedics, Paramedics, Pharmacists and Healthcare Assistants. Urgent Response teams work closely with Intermediate Care staff to provide support to keep people at home wherever possible

Main duties of the job

Your main duties include:

5. Comprehensive specialist assessment, review and evaluation of the needs of patients and carers within a 0-2 hour, 2-24 hour timeframe
6. Being responsible for undertaking an urgent community response to people in their own homes to prevent hospital conveyances and keeping patient safe at home with the appropriate clinical interventions.
7. Working with a range of professionals in order to ensure care is appropriate and delivered in a timely manner.
8. Demonstrating professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery.

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria

18. Demonstrate profession specific clinical assessment planning delivery and evaluation skills.
19. Excellent written and verbal communication skills
20. Presentation skills
21. Self-Awareness
22. Ability to motivate staff

Desirable criteria

23. Ability to manage complex cases
24. Ability to develop and maintain partnership working
25. Knowledge of legal requirement for working with vulnerable adults and those who lack capacity
26. Knowledge of modernisation agenda within the NHS and social care
27. Report writing skills
28. IT Skills
29. Teaching patients, carers and colleagues

EXPERIENCE

Essential criteria

30. Demonstrate experience post or preregistration placements in relevant clinical areas for therapists:- Care of Elderly Orthopaedics Neurology Respiratory Palliative Care Long term conditions
31. In depth experience of working within multi-agency/multi-disciplinary team

Desirable criteria

32. Experience of working in a community/in-patient setting for a minimum of 2 years post registration
33. Experience of electronic clinical systems
34. Experience participating in research or audit project
35. Experience of day-to-day staff management e.g. case allocation to Health Care Assistants
36. Experience of working with single assessment process

SPECIAL CIRCUMSTANCES

Essential criteria

37. Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria

38. Registered Allied Healthcare Professional with HCPC - Physiotherapist Occupational Therapist
39. Degree/ Diploma in Physiotherapy Occupational Therapy Nursing Paramedic Science
40. Relevant post graduate clinical training e.g. Advanced History Taking & Assessment Long Term Conditions Respiratory Disease Cardiac Disease Trauma, Orthopaedics & End of Life
41. Experience of recent Continuous Professional Development

PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate personal protective equipment (PPE) in accordance with our infection prevention and control procedures.
