Job Title: Social Worker – Preparing for Adulthood – Learning Disabilities & Autism
Location: Rochdale
Pay Rate: £35 per hour
Liquid Personnel is recruiting an Adult Social Worker to join its client’s Preparing for Adulthood – Learning Disabilities & Autism Team, based in Rochdale.
What will your responsibilities be?
1. Conducting Care Act assessments: Evaluate young people (usually 17+) to determine eligibility for adult social care services.
2. Coordinating transition planning: Work with schools, families, and professionals to ensure smooth movement from children’s to adult services.
3. Supporting independence and community inclusion: Help individuals develop daily living skills, access housing options, and engage in employment or training pathways.

Qualifications and Experience:
To be successful in this role you must have.
7. Social Work England registration
8. Eligible to work in the UK
9. Hold a full UK driving licence
10. Post-qualified experience
11. Degree level or equivalent in Social Work
