An excellent opportunity has arisen for a Band 4 Multi-Therapy Assistant to join a friendly, supportive, and well-established integrated community team within Berkshire Healthcare NHS Foundation Trust (BHFT). This is an exciting time to join the service as we continue to develop and strengthen integrated ways of working across West Berkshire and Berkshire West.
The successful candidate will work alongside a knowledgeable and experienced multi-disciplinary team, including Senior Specialist Community Physiotherapists, Occupational Therapists, Therapy Assistants, and administrative colleagues. You will be an integral member of the Clinical Support Team, contributing to the delivery of high-quality, responsive, and person-centred care in the community.
Our integrated service supports a range of pathways including Community Reablement, Falls Prevention, and Community Physiotherapy, working closely with health, social care, and voluntary sector partners. This role involves community visits, offering variety and autonomy, with agile working opportunities to support work-life balance, and development will be supported through supervision and training.
Main duties of the job
You will support adults with a wide range of physical health conditions, functional impairments, and mental health needs, helping people to maximise independence and remain safely at home.
Work as part of the multi-disciplinary team to support the assessment, treatment, and rehabilitation of patients across all service pathways.
Contribute to discussions regarding patient progress and outcomes, providing feedback to the wider team and participating in reviews as appropriate.
Carry out therapy and exercise programmes as advised by registered clinicians, supporting patients to improve mobility, function, and activities of daily living (ADLs).
Maintain a patient-centred approach, promoting dignity, independence, and choice in all interactions.
Liaise effectively with Registered Nurses, Occupational Therapists, Physiotherapists, Care Managers, and Social Workers, ensuring accurate, timely, and concise documentation.
Participate in team meetings, supervision, and professional development activities, demonstrating a commitment to learning and service improvement.
Comply with Trust policies, safeguarding responsibilities, and information governance requirements at all times.

* NVQ4/ Or Equivalent
* Basic computer course
* ECDL
Continuous Professional Development:
* Evidence of on-going professional development/relevant courses

Previous Experience:
* Minimum of 3 years community /nursing /care experience.
* Palliative Care Experience
* Experience of caring for the Elderly
* Proven working experience of one or more of the following: Intermediate Care Assistant Experience, Health care assistant experience, Occupational therapy assistant experience or Physiotherapy assistant experience
* Experience of shift lead.
Knowledge, Skills & Abilities:
* Ability to deal with and communicate sensitive or difficult issues over the telephone or face to face.
