Our Home First & Reablement service supports people to return home after a stay in hospital, helping them to recover in their own surroundings and regain the ability to live as independently as possible.

Responsibilities
* Work closely with other health care professionals, delivering essential clinical and personal care to service users.
* Ensure that service user needs are met and assist the team in maintaining high standards of care delivery.
Qualifications, Training & Experience
We are recruiting both experienced and new to care Rehabilitation Assistants. If you are new to care and hold no experience in healthcare, you will be offered a structured development plan that includes achieving the Care Certificate. Successful completion will allow progression from pay band 2 to pay band 3.
Hours & Shifts
Our service runs 24/7 and all Rehabilitation Assistant roles offer core hours of 07:00–22:00.
The following shifts are currently highly sought after for this locality:
* PM – 17:00 to 22:00 (4 on 4 off)
All applicants must have a valid UK driving licence and access to a vehicle that they can use for work purposes, as the role is locality‑based visiting people in their homes.
