Offer description

To support individuals referred to secondary mental health services for assessment or ongoing care. To demonstrate the Trust values of inspiring hope, working together, to be caring and compassionate, to value and respect others/ourselves and to be open and honest.

As a Registered Mental Health Nurse you will demonstrate and live the NMC values as identified in the relevant Code of Practice.

Your post will include working with individuals using evidence‑based interventions to promote their independence and recovery whilst maintaining high quality clinical documentation.

This includes respecting individuality, values, cultures and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.


Main duties of the job

* To promote positive mental and physical health to individuals under the care of the service in all situations.
* To support individuals to collaborate in the development of their recovery plans and structured care plans and treatment, working to build a strong therapeutic alliance.
* To assess, plan and review in line with requirements of the Care Act (2014) and s117 of the Mental Health Act (1983).
* To provide clinical supervision to members of the multi‑disciplinary team based on your clinical and professional knowledge.
* To supervise junior members of staff in their clinical risk assessment and formulation of crisis and risk planning.
* To promote recovery‑focused care and interventions for individuals.
* To ensure that all interactions and documentation is person‑centred in line with NHS guidance “No decision about me, without me”.
* To co‑produce individualised recovery plans with the individuals, carers, and others identified by the service user.
* To promote and maintain a learning culture within the team which supports the professional and effectiveness of clinical care.
* To contribute to the delivery of multidisciplinary training including the induction of new staff, students and trainees.
* To always promote awareness of, and respect for, equality, diversity and dignity in care and work in line with Trust and professional standards.
* To demonstrate a high level of active communication skills with individuals in a variety of forums and situations.
* To utilise your highly developed interpersonal skills to facilitate effective communication, identifying and overcoming communication barriers such as sensory or cognitive impairment, hostile or highly emotive situations.
* To provide specialist mental health interventions in the assessment, support and recovery of individuals with severe mental health difficulties (including Mental State assessments, Social Care Assessments, physical health care monitoring, side‑effect profile monitoring, etc.).
* To ensure that all individuals have clear and accurate information regarding their care or the care of their loved ones.
* To act up in the absence of the senior practitioner, if required.
* To seek out and participate in line management and clinical supervision.
* To maintain high standards of clinical recording ensuring that information is always accurate, appropriate and up to date.
* To comply with Greater Manchester Mental Health NHS Foundation Trust policies and procedures, with a specific focus on confidentiality, Information Governance, and professional practice.
* To maintain your personal well‑being using all available resources provided by the Trust.
* To comply with all health and safety legislation and the welfare of yourself and the wider Trust.
* To identify unmet needs to support service improvement or planning to enhance individuals’ experiences of health services within the Trust or in the wider health economy.
* To adhere to the principles of safeguarding children and vulnerable adults inline with Trust policy and local and national reporting procedures.
* To act inline with the Trust’s Infection Prevention and Control policies, procedures and best practice guidance.
* To comply with all service security requirements regarding your own and others’ safety, including lone working procedures, health and safety at work and risk management procedures.
* To ensure the management of risk in accordance with Trust policies and procedures, including the investigation process for Root Cause Analysis, complaints, accidents and near‑miss situations via the InPhase system.
* To uphold the Trust values and work in an environment free from harassment, discrimination, abuse and violence.
* To promote the Trust’s anti‑racism agenda in all professional and personal interactions.
* To develop and maintain good working relationships with all partners involved in individuals’ care or the wider health economy.
* To promote awareness of mental health and the role of the CMHT within local services and the wider community.
* To advocate on behalf of individuals under your care, ensuring they are fully involved with their care and treatment.
* To exercise excellent time management, punctuality, consistency and reliability within your role.
* To demonstrate empathy and compassion to individuals or their family/carers who may want to make a complaint about their care.
* To take responsibility for your own learning and professional development, making use of opportunities to expand your clinical, professional or governance skills.
* To act as an effective and empowered role model for others, sharing your knowledge base and clinical skills.
* To act as a Practice Educator for students on clinical placements.
* To maintain your professional registration and inform the Trust of any changes, conditions or restrictions on your registration.
* To use access to clinical information only where required for your duties.
* To maintain clinically sensitive information confidentially in line with GDPR and only communicate it where authorised.
* To understand and comply with all Trust policies, procedures, protocols and guidelines.
* To understand the Trust’s Strategic Goals and how you can support them.
* To carry out all duties and responsibilities of the post in accordance with equal opportunity, equality and diversity and dignity in care/work policies and principles.

Person Specification

* NMC Registered.
* Experience working in a CMHT.
* Professional qualification RMN.
* Willingness to participate in your own continuing professional development relevant to the role.
* Registered with the NMC.
* Completing assessments using a biopsychosocial framework.
* Completing s42 enquiries.
* Completing Mental Capacity Act assessments.
* Participating in multi‑agency meetings.
