Key Duties
Medication Reviews: Conduct comprehensive medication reviews for patients with long-term conditions, optimizing their treatment plans and improving medication adherence.Chronic Disease Management: Work with GP practices to manage chronic conditions such as diabetes, asthma, hypertension, and cardiovascular disease, ensuring the safe and effective use of medicines.Patient Consultation: Provide expert advice to patients and healthcare professionals regarding the use of medications, side effects, and potential interactions.Clinical Advice: Offer evidence-based clinical advice to GPs, nurses, and other healthcare professionals, helping to ensure the best treatment outcomes for Promotion: Play an active role in promoting preventative care and healthy lifestyle choices, including smoking cessation, weight management, and vaccination programs.Medicine Optimisation: Review and manage prescribing patterns within the practice to ensure that medicines are used safely, effectively, and Collaboration: Work closely with a range of healthcare professionals to ensure a holistic and patient-centred approach to and Compliance: Undertake audits of medication use and prescribing patterns to support the improvement of clinical practices and ensure adherence to local and national guidelines.Support and Education: Provide education and training to staff on medication-related topics and support the development of clinical pathways and guidelines.

Key Skills and Qualifications
