Experienced Social Worker - Clatterbridge Intermediate Care Centre (CICC)
Wirral Council invites applications for a dedicated and compassionate Experienced Social Worker to join our CICC team, playing a pivotal role in delivering personalised, strength‑based support for adults and carers across the borough.
About the role
This role is split between office bases at Pennant House and in‑reach support to the Clatterbridge Hospital Reablement Wards, offering an excellent development opportunity for experienced social workers who wish to broaden their skills. You will work within a dynamic, multi‑disciplinary team, supporting safe, timely discharges from CICC while promoting independence, well‑being and positive outcomes.
Key responsibilities
* Deliver high‑quality health and social care functions for adults and carers, ensuring practice reflects the Care Act 2014, strength‑based working, personalisation and social inclusion.
* Work with adults with physical health needs, mental health needs, older adults and carers, often those experiencing complex circumstances.
* Support the development of others, including ASYE/newly qualified social workers and Care Navigators, modelling good practice and championing professional standards.
* Plan, assess and safeguard individuals, working within statutory frameworks and multi‑disciplinary teams.
* Maintain high‑quality practice, prioritise demanding workloads and manage complex and sensitive situations.
Key qualifications
* Social Work qualification required (Degree in Social Work, Dip SW or equivalent) and registered with Social Work England.
* Strong background working with adults with complex needs, carers and families.
* Skilled in assessments, support planning, safeguarding and working within statutory frameworks.
* Excellent written and verbal communication skills.
